Now, new research presented at the American College of Cardiology's Annual Scientific Session (ACC.26) adds a critical layer of nuance: it's not just how much you drink, but what you drink — and how you drink it.
The Study: 340,924 People, 13+ Years
Led by Dr. Zhangling Chen of the Second Xiangya Hospital, Central South University in China, the study drew from the UK Biobank — one of the world's largest biomedical databases. Researchers tracked drinking habits and health outcomes across 340,924 adults for an average of more than 13 years, making it one of the most comprehensive looks at beverage-specific cardiovascular risk to date.
Participants were grouped by both alcohol type and consumption level: low-to-moderate (up to ~1.5 drinks per day for women, ~3 for men) and high (above those thresholds). The results? The relationship between alcohol and heart health is far more granular than blanket guidelines suggest.

Moderate wine drinkers showed a significant reduction in cardiovascular mortality compared to those who rarely or never drank.

Wine: The Outlier in the Data

Moderate wine drinkers showed a 21% reduction in cardiovascular mortality compared to those who rarely or never drank. Meanwhile, the same consumption level of spirits, beer, or cider was associated with a significantly higher risk of death — even at low intake.
The contrast is striking: low intake of beer, cider, or spirits was linked to a 9% higher risk of dying from cardiovascular disease compared with never or occasional drinkers. Wine, at equivalent volumes, moved in the opposite direction.

Why Might Wine Be Different?
The researchers point to three interlocking factors that may explain wine's divergent risk profile:
- Polyphenols and antioxidants. Red wine contains bioactive compounds — notably polyphenols like resveratrol — that have been linked to improved vascular function and reduced inflammation. These compounds are largely absent in distilled spirits and present in lower concentrations in beer.
- Meal-time consumption. Wine is far more likely to be consumed with food, which slows alcohol absorption, reduces blood sugar spikes, and is associated with better metabolic outcomes. Spirits, beer, and cider are more commonly consumed outside of meals.
- Lifestyle clustering. Wine drinkers in the study tended to have higher-quality diets and healthier behaviors overall. The beverage choice may be a marker for broader lifestyle patterns — though researchers attempted to control for these confounders.
The Heavy Drinking Warning
Whatever advantage wine may hold at moderate levels, it evaporates — and reverses — at high consumption. The study found that heavy drinkers across all beverage types faced substantially elevated risks:
- 24% higher risk of death from any cause
- 36% higher risk of cancer death
- 14% higher risk of heart disease death
The message is unambiguous: moderation is not a suggestion. The cardiovascular benefits observed in moderate wine drinkers do not scale with volume. There is no "more is better" here.

Before anyone rushes to restructure their drinking habits, several critical limitations apply:
Observational, Not Causal
This study identifies associations, not cause-and-effect relationships. It was based on self-reported drinking habits at a single point in time and did not capture changes in behavior over the 13-year follow-up period.

These findings were presented at ACC.26 in March 2026 but have not yet been published in a peer-reviewed journal. Conference presentations are considered preliminary until they undergo full academic review.
Don't Start Drinking for Your Heart
Major health organizations including the American Heart Association and the Mayo Clinic explicitly advise that people who do not currently drink alcohol should not start in order to gain cardiovascular benefits. The risks of initiating alcohol consumption outweigh the potential protective effects for non-drinkers.
